How can I get in touch with a contractor that did some work for me last year?

A roofer put shingles on a dormer. Now the roof is leaking where he worked. I tried calling the number on his business card apparently it is out of service now. I tried to find his address, but the number of the location indicated does not exist (e.g. 169 Maple St., but the numbers on Maple St only go up to 150). Suggestions?
